Medication Authorization Form

Required for any medication administered during childcare.

Allergy & Dietary Information Form

Used to notify staff of food allergies, dietary restrictions, and medical needs.

Animal Acknowledgement Form

Information regarding our classroom dog and hermit crabs.

Child Health Examination Form/ Immunization Records

Required by New York State for enrollment. Please provide updated immunization records as required by New York State regulations.

Orleans County Department of Social Services

Families may qualify for child care assistance to help cover the cost of care.

Early Intervention (Birth–3 Years)

If you have concerns about your child's development, Early Intervention services may provide evaluations and support services for eligible children.

Preschool Special Education Services (Ages 3–5)

Children ages 3–5 may qualify for evaluations and support services through their local school district.

Child Care Resource & Referral (CCRR)

Provides information about child care assistance programs, parenting resources, and community services.

Nutrition & Healthy Eating

At Mother Daughter Daycare, we believe healthy eating plays an important role in a child's growth, development, and ability to learn. We strive to provide nutritious meals and snacks that support healthy habits and introduce children to a variety of foods.

Child and Adult Care Food Program (CACFP)

Our center participates in the Child and Adult Care Food Program (CACFP), a federally funded program that helps ensure children receive nutritious meals and snacks while in care.

Through CACFP, meals are planned to meet USDA nutrition guidelines and support healthy development.

🌽 Community Supported Agriculture (CSA)

We proudly participate in a Community Supported Agriculture (CSA) partnership with Panek's Pickin' Patch.

This partnership allows us to incorporate fresh, locally grown fruits and vegetables into our menus whenever possible. By connecting children with fresh seasonal produce, we help encourage healthy eating habits while supporting local agriculture.

Children may have opportunities to:

  • Explore new fruits and vegetables

  • Learn about where food comes from

  • Participate in nutrition-related activities

  • Experience seasonal foods grown in our local community
